    Yes.

    A one time notice of intent must be filed within 30 days of beginning to homeschool.

    It must have the child’s name, date of birth, address of the school they were attending and the names, phone numbers and addresses of the parent or guardian with custody.

    The notice also requires a copy of the child’s birth certificate.
